Send the reminder from the record, or not at all
The test is whether cancelling an appointment cancels the reminder that was queued for it. If the message list is a separate system fed by an export, that will fail the first week, and the failure is visible to your clients rather than to you.
Look at what the client actually receives
Ask to see the real template, not a description of it: the sender address, whether it carries your practice name or the vendor's, and whether a client can reply to it. A reminder from noreply@vendor.example is a reminder from somebody your client has never heard of.
Decide what you will never send
Recalls and appointment reminders are service messages your clients expect. Promotions are not, and the consent rules differ by jurisdiction. Decide the line before you buy, because a system that makes the two the same list will eventually send the wrong one.
